Mid-Missouri Premier League - Futsal
REGISTRATION DEADLINE: DECEMBER 5th / **********Mail payments to: MMPL Futsal c/o Carrera Soccer Club 503 E Nifong Blvd #214 Columbia, MO 65201 / **********Fees: $62 per game (2-6 games per team) Plus $5.00 per player for the MYSA fee **********Games will be played in Columbia & Boonville **********Game dates include: January 8, 15, 22, 29, February 5, 12, 19, 26 **********You must enter the number of games and the number of players playing in order for your application to be complete. You can add player names at a later date. **********PAYMENT IS DUE DECEMBER 5TH!!!
