Registration

What you will need for registration: Waiver of Liability signed by all parents and coach's, 2 copies of your State Roster, Player Cards (no copies must be orginal), Medical releases for all players, Guest Player form if you are using any guest players that are not listed on your state roster., If you are traveling from outside of Indiana you will also need to bring your premission to travel form given to you by your state commissioner.
